At Highfield Surgery, we are committed to respecting and supporting your right to update your personal information on our records, including your name or gender. Below is a guide for any patient wishing to notify us of changes:
If you are aged 18 or over and wish to update your gender on your GP record, please follow the steps below:
Please complete the new patient registration form (PRF1) with your new gender, even if you are already registered with our practice.
Email the completed form to: highfield-surgery.noreply@nhs.net, and clearly state that you are requesting a gender reassignment.
Once submitted:
A new NHS number will be issued for you.
Your existing medical records linked to your old NHS number will be securely transferred by Primary Care Support England (PCSE) to your new NHS number.
Please note:
If you are under 18, please contact us before you take any steps.
For confidentiality and information governance, we will remove all references to your previous gender from your records.
If you would like to change your name—whether it is your first name or surname—and it differs from the name shown on your photo ID currently held in your medical record:
Patients over 16 should provide a formal or self-declared deed poll with the correct legal wording.
The document must be scanned in full (screenshots or partial pages will not be accepted) and send it to highfield-surgery.noreply@nhs.net.

Please note:
The deed poll is your official proof of name change for use across other organisations.
We do not provide GP letters to support your name change requests for use with external organisations. Your deed poll serves as your official proof of name change and should be used for updating your name with other organisations, rather than relying on your medical record.
Once we receive and verify your document, we will notify NHS on your behalf to change your offical name.
