Social Prescribing Link Worker

Social Prescribing Link worker or called social prescriber, is a member of a primary care network (PCN) team who helps people access local services to improve their health and wellbeing.

Umesh Patel (m)

First Contact Physiotherapist (FCP)

Experienced physiotherapist who is specially trained to assess, diagnose, and manage musculoskeletal (MSK) conditions. Patients with joint, muscle, or bone pain can see an FCP directly without needing to see a GP first.

FCPs provide expert advice on how to manage conditions such as back pain, sprains, or arthritis, and can refer patients for further investigations or treatments if needed. They help reduce waiting times and ensure patients receive the most appropriate care as early as possible.
